Course Registration Form

This form may be used for registrations by Gold and Gold Plus One members and for guests and non-members registering for free classes and lectures. Please note that many classes are open only to Osher members.

Course location details will be provided in a confirmation sent via e-mail (or mail if no e-mail is on file) one week before the start of the course. If you require special accommodations to attend a class, please notify the Osher Lifelong Learning Institute at least 15 days in advance.

Review the course details found on the Schedule of Classes for title and start date. Use this form to register for up to ten courses. Be sure to include start date for each course selected. At least one course must be listed.

If you would like to register for more than ten courses, you may return to this page to complete this form again. Your Gold or Gold Plus One membership entitles you to register for as many Osher Institute classes as you'd like.
