Evening School (Undergraduate & Graduate)

Application Form

New student information form for the Evening School. Prospective students
enrolling in classes for enrichment only should complete the top half
of the form; prospective students applying for admission to a degree
or certificate program should complete the entire form.

Application Form

Application form for graduate study toward certificates or masters
degrees. Prospective graduate students should complete the entire
form, including the required attachments, and submit a nonrefundable
$50 application fee. Also available in MS Word format.

Recertification Student Information Form

Application and registration form for students taking educator recertification
courses from the "Reaching Out to Educators" brochure. Also available in MS Word format.

Declaration Form - Non-ESM Minors

Evening school students in all programs EXCEPT Emergency Services
Management who wish to pursue a minor should complete this form and
submit to the School of Continuing Studies office.

Declaration Form - ESM MINORS ONLY

Evening School students pursuing a minor through the Emergency Services
Management program should complete this form and submit to the School
of Continuing Studies office.
- Vehicle
Registration Form

Evening School students should register their vehicles with Parking
Services before classes begin. Parking fees will be charged to
your tuition account.
- Osher Reentry Sholarship Fund

Available to undergraduate students who:
- Have interrupted their collegiate studies for approximately 5 or more years before being admitted to SCS
- Look foward to a significant number of years of workforce participation after graduation
- Are between the ages of 25 and 50
- Seek their first baccalaureate degree (and are admitted to a baccalaureate degree program in SCS)
- Demonstrate financial need as reported on the scholarship and in the letter submitted by each student
- Make satisfactory academic progress in SCS (maintaining a 2.0 GPA)

H. Proffitt Scholarship

Available to bachelor degree candidates who have been accepted to
the Evening School. This scholarship is administered by the School
of Continuing Studies Alumni Association.
- Need-Based
Scholarships

Available to certificate, associate and bachelor degree candidates
who have completed at least 15 semester hours in the Evening School.
These scholarships are need-based and are not available to students
who receive other financial assistance.
