Faculty and Staff Forms

The following forms are available to faculty and staff for various administrative tasks.

Information Update & ACE Form
Update contact information and/or describe Accomplishments, Congratulations, and Excellence. Available as a PDF Document or Microsoft Word form.

Faculty Personal & Professional Development
Use this form (PDF Document or Microsoft Word form) to make the following requests. Complete form in advance of the activity or event for approval and provide appropriate follow up paperwork.

• Professional and Personal Development ($100 per academic year) for a course, conference, class teaching aid, activity, or fees for the Weinstein Center for Recreation and Wellness (usage fees are assessed for adjunct faculty who are not full-time)
• Instructional Support/Social Fund ($50 per course) for social and/or instructional support

Staff Personal Wellness & Professional Development
Use this form (PDF Document or Microsoft Word form) to make the following requests. Up to $100 per fiscal year is available. Complete form in advance of the activity or event for approval and provide appropriate follow up paperwork.

• Personal Wellness for Weinstein Center for Recreation and Wellness fees (usage fees are assessed for part-time staff)
• Professional Development for appropriate activities completed by full-time or part-time staff members

Guest Speaker Approval Form
Use this form (PDF Document or Microsoft Word form) exclusively for guest speakers. This form replaces the old "Instructional Support Services Contract," so you need only complete this single form. Follow instructions listed with the form.

Textbook Ordering Form
Request an Instructor's Desk Copy or Possible Adoption Text. Available in PDF or Word format.

Bookstore Textbook Requisition
Use this online form to request student books for the University bookstore.

Non-Credit Tuition Remission Application
University of Richmond faculty and staff may download the Tuition Remission Application Formf in PDF format or Word format and submit with this registration.
